Initial Comments:
Based on the findings of an unannounced off-site follow up survey, conducted on December 3, 2021, found that Bethlen communities Hospice had corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a Medicare recertification survey completed August 31, 2021.
